$500,000 up to $2,000,000 general liability coverage per occurrence. "Occurrence"
means any accident, including continuous or repeated exposure to substantially
the same general harmful conditions.

| How Insurance Commences |
|
Coverage
will be bound upon receipt and approval of completed enrollment form
and payment of appropriate premium. You will then be provided a Certificate
of Insurance showing proof of coverage. |
